Admission for College

Section 1. Admission Requirements

1.1 Freshmen

a. Form 138 (1 set original, 2 sets photocopy)

b. Senior High School Diploma (photocopy)

c. Two 2x2 colored photo

d. Birth Certificate (photocopy of PSA authenticated copy)

e. Certificate of Good Moral Character

f. Clearances (Barangay, Police, and Medical)

g. Entrance Exam Result


1.2 Transferees

a. Honorable Dismissal (1 set original, 2 sets photocopy)

b. Photocopy of Transcript of Records

c. Two colored 2x2 photo

d. Birth Certificate (photocopy of PSA authenticated copy)

e. Certificate of Good Moral Character

f. Clearances (Barangay, Police and Medical)

g. Entrance Exam Result

 

Freshmen and Transferees must obtain at least 30% rating for BSBA and BSIT and 40% for Education as a requirement for admission to the Community College.

1.3 ALS PASSER

a. PSA Birth Certificate

b. Certification

c. Certificate of Good Moral Character

d. Certificate of Rating

e. ALS Diploma

f. 6 pcs 2x2 ID Picture (Studio taken and with collar and in white background; must be taken within the last 6 months

g. PSA Marriage Certificate (For married women)

h. Police Clearance for School  Purposes

i. Medical Certificate for School Purposes

j. 3 pcs Long Brown Envelop with Plastic Cover

k. 3 pcs 1/2 size index card

l. Vaccine Card / Vax Certificate (3 pcs photocopy)

 

 

Section 2. Admissions Procedure

2.1 Freshmen and Transferees

Step 1 Check the schedule for Entrance Examination

Step 2 Secure the necessary documents for the Entrance Exam

Step 3 Proceed to the Finance Office and pay the Entrance Exam Fee

Step 4 Proceed to the Testing Office and take the Entrance Exam

Step 5 Verify Entrance Exam Results with the Test Administrator

Note: Entrance Exam Fee is non-refundable.

Section 3. Enrollment Procedure

3.1 Freshmen and Transferees

Step 1 Proceed to the Faculty Office/Office of the Program Head for the submission of school credentials (photocopies) for evaluation.

Step 2 Proceed to the Registrar for Enrolment Validation

Step 3 Proceed to the Computer Laboratory for ID Processing
